Tragic Suicide Could Have Been Prevented

I was saddened to hear that Damon, 24 years old, was let down so badly by those treating him. Damon was found on the street; having cut his wrists under the influence of alcohol.
The Northumberland Tyne and Wear NHS Foundation Trust Crisis Team carried out an assessment, and even though Damon had a number of financial and social problems, he was discharged with no follow up. Two days later he committed suicide.
This is a tragic death which could have been prevented. Not only was the care given to Damon appalling but so was the handling of Mrs Cook’s subsequent complaint. It is frustrating to see that it took the Trust over two years to apologise for their failings.
